Connection & Collaboration:
Innovations in Patient-Centered Care

At the heart of holism is an understanding of connection.
Collaboration is key to bringing our collective resources together
for the greater good of the patient and society.
We will look at collaborative models of care within and between
clinics, collaborative views such as the role of the human biome in health
and disease states, research models such as case studies that are the
legs of change, multi-pronged, team approaches to care and the role
of mindfulness in creating connection within and without.
